Primary Care Partnerships were first established in 2000. Over the past fifteen years, we have led and facilitated many partnership initiatives across a broad range of health and community organisations operating in the inner east.
These stories include our work in developing the Social Inclusion Framework, building capacity of organisations to understand health literacy, creation of The Well, how the Opening Doors program was conceived, our role in improving the integration of services for people with diabetes, and Chasing the Luck? a co-design project with the Chinese community.
