The Inner East Primary Care Partnership made a difference to the health of the inner east community by leading, collaborating and advocating for strong partnership work across the region, supporting organisations with information and resources to improve evidence-based practice, and by working closely with government in order to align regional priorities for collective impact.
Our key priorities were identified through our strategic planning process as important issues within our community. These priorities address the major challenges in our region:
- Prevention and Population Health
- Healthy Ageing
- Working with Communities
- Social Inclusion
By bringing organisations together to respond to shared issues and priorities we were able to have a bigger impact. This involved leading the work and acting as the backbone organisation or, as with our work in the Prevention of Violence against Women, supporting an existing partnership by bringing our particular expertise to the table.
Work on each priority was informed by rigorous needs analysis, collaboration, and ongoing consultation with our partner agencies.
