To Foster and Facilitate a Stronger Prevention and Health Promotion Focus Within System and Beyond

Collaborative Planning Project:
Development of a Collaborative Planning Framework in the Inner East to support the development of Municipal Health and Wellbeing Plans, Community and Women’s Health, Health Promotion Plans.
Continued support for local government, community and women’s health with planning and evaluation of their prevention strategies.

The Well:
In collaboration with the Outer East Primary Care Partnership
Continued support for the development, and uptake of “The Well” website to support collaboration and evidence based practice.

Health and Wellbeing of Older People:
In collaboration with the Outer East Primary Care Partnership
Development of a comprehensive profile of the Health and Wellbeing of Older People across the Eastern Metropolitan Region as the first step in identifying common areas of need and opportunities for collaborative action.

Supporting and/or leading collaborative prevention work on a range of priority issues:
Food Access and Security, Prevention of Violence Against Women, Drugs and Alcohol, Improving Social Connectedness

 

Facilitate a Better Consumer-Driven and Consumer-Centred System

Improving Health Literacy across the Eastern Region:
In collaboration with the Outer East Primary Care Partnership
We provide training and professional development to partner agencies across the catchment to strengthen strategic approaches to improving health literacy.

Koolin Balit:
Health and wellbeing services supporting Aboriginal tertiary and secondary students in the Inner East
Identifying opportunities to strengthen pathways to provide support for Aboriginal tertiary students between their institutions and local health and wellbeing services.

 

Facilitate a Connected, Efficient, Robust and Accessible Primary Health Care and Community Service System

Gestational Diabetes Project:
with Eastern Health and Carrington Health
Initial research will focus on understanding the service demands and pressures and identifying key opportunities to improve the prevention and care of women with gestational diabetes.

Service Coordination Practitioner Network:
Supporting community aged care service providers and supporting change management.

Participation in the Eastern Health Primary Care Collaborative

 

Strengthen the Capacity of the PCP

  • Review and strengthen the governance of IEPCP including simplifying the membership process
  • Improve IEPCP marketing, branding and communications
  • Identify strategic opportunities in a changing external environment
  • Focus on partnership development with Eastern Melbourne PHN and the Primary Care Partnerships within this catchment to ensure we work effectively together
