Early Intervention and Integrated Care

Overview

Early Intervention and Integrated Care is a priority area for the Inner East Primary Care Partnership. The aim of our work is to strengthen the health and community services system to deliver person-centred and accessible care in a timely and coordinated way. We want to keep people as well as possible for as long as possible, especially if they have multiple and complex needs.

We work to support our partners by facilitating collaboration, networks, and working groups; promoting best practice and innovation; providing information and resources; and building the capacity of practitioners and service providers to meet the needs of their consumers.

Some key areas of our work are:

  • Service Coordination –strengthening the capacity of agencies to provide quality care coordination for their consumers
  • Chronic Disease – supporting the development of information and linkages to local community resources on the HealthPathways Melbourne website and improving care for people with diabetes
  • Older people –improving our understanding of, and collaborative response to, the health, wellbeing, independence and safety needs of older people in the Eastern Metropolitan Region
