Our Success Stories

Primary Care Partnerships were first established in 2002. Over the past fifteen years we have led and facilitated many partnership initiatives across a broad range of health and community organisations operating in the inner east.

These stories include how the Opening Doors program was conceived and developed, our role in improving the integration of services for people with diabetes, improving the understanding of the needs of carers and people with a chronic illness and the supports available to them through the Community Health Pathways project, and Chasing the Luck? a co-design project with the Chinese community.


We acknowledge the Wurundjeri people and other peoples of the Kulin nation as the traditional owners of the land on which our work in the community takes place. We pay our respects to their Elders past and present.
