Practitioners Network

Overview

The Inner East Primary Care Partnership hosts the Service Coordination Practitioners’ Network.

The Network is for health and human services practitioners to share their experiences, hear about new initiatives, exchange information, encourage best practice and provide an opportunity to work in partnership to improve outcomes for clients.

Regular meetings are held with guest speakers plus information updates are provided to the membership. A small secretariat including the Network Chair, Inner East PCP Integrated Care Coordinator and two other network members, organises each meeting.

Why run a Service Coordination Practitioners’ Network?

The Inner East PCP Service Coordination Practitioners’ Network has a focus on the Inner East PCP strategic priorities of Healthy Ageing, Mental Health, Disability and Chronic Conditions. The purpose of the Network is to:

  • Connect people who work in the field
  • Address issues that affect your work and your organisation and
  • Provide information about the services and activities your organisation offers
  • Provide an opportunity for communicating, sharing experience and building insight and understanding
  • Stimulate learning through guest speakers, information exchange, mentoring, coaching and self-reflection
  • Generate new knowledge to help people towards best practice and to accommodate changes in needs, pressures and technology
  • Better meet the needs of clients. 

Become involved

Network Membership

Membership of the Network is open to all practitioners working in the delivery of health care and/or human or social support services in the Inner East. This includes Boroondara, Manningham, Whitehorse and Monash. The Network meets every second month. You can attend meetings and/or be placed on a mailing list to keep in touch with what is happening.
