Health Pathways

Overview

HealthPathways is a web-based system for General Practitioners (GPs) that helps coordinate patient care across the acute and primary care system. The pathways provide guidance for GPs when seeing patients for assessment, management and referral for common medical conditions.

The Inner East Primary Care Partnership is collaborating with Health Pathways Melbourne to develop and review community resources to further support GPs in the care of their patients in Melbourne’s North West and Eastern regions.

The Inner East PCP HealthPathways Melbourne Community Resources Pilot Project 2015 was a collaboration with HealthPathways Melbourne to include community resources and other information about services to meet the needs of patients with chronic and complex health needs. The project involved extensive consultation with carers, consumers and health professionals to develop high quality resources for the HealthPathways website.

Following on from this successful HealthPathways Project, the Inner East PCP will continue to review and update community resources and support service information developed, and look at options to develop new resources to support GPs and their patients, such as increased information about community health and local government services.

Why is HealthPathways a priority?

HealthPathways Melbourne aims to:

  • Enhance clinical knowledge and promote best practice care
  • Build collaboration and reduce fragmentation across the health service network
  • Reduce the number of patients referred to specialist care who could be managed in a primary/community care setting.

Inner East PCP work on the HealthPathways Melbourne project:

  • Demonstrates how our skills in integrated care compliment HealthPathways work
  • Draws on and strengthens our close working relationships with a wide range of community organisations to add “the next layer” to the information provided on HealthPathways
  • Improves the coordination of services and chronic disease management for people living with chronic and complex conditions and their carers

Become involved

Inner East PCP partners can be involved in HealthPathways Melbourne by providing support to review and develop new resources through working groups and consultations. We also encourage practitioners to register to access HealthPathways Melbourne and provide feedback about any updates or changes required to the community resources.
